Your application for a Visit Pass (Long Term) has been approved in principle (pending payment), subjected to the following conditions:
- the validity of the travel document;
- a valid Visit Pass;
- the results of applicant's medical report, (not applicable to an applicant below 15 years old);
- S$30/- visa fee and S$60/- issuance fee ;
and that you are able to furnish the following documents:
- A copy of duly completed Form 14.
- Applicant's passport.
- Applicant's DE Card
- Sponsor's NRIC(Original)
- Sponsor's birth certificate showing the relationship between the applicant and sponsor (Original and Photocopy)
- Sponsor's latest letter of employment stating date of commencement, designation and salary per month. This letter submitted to ICA should be within one month from the date of issue;(Original and Photocopy)
- Sponsor's payslips for the last 6 months (Original and Photocopy)
- Sponsor's CPF Statement showing monthly CPF contribution (My Statement - Contribution History) for the last 12 months;
- 1 recent passport sized colour photograph which meets the following requirements to be pasted on the top right-hand corner of Form 14:
- The photograph image must be taken within the last 3 months;
- The photograph should be in colour, must be taken against a white background with a matt or semi-matt finish;
- The photograph image must show the full face and without headgear (headgear worn in accordance with religious or racial customs is acceptable but must not hide the facial features);
(The original and photocopies of the above documents are required)
